Although we primarily offer face-to-face appointments, the COVID-19 Emergency Rule has temporarily allowed video/audio encounters for the duration of the pandemic. Our providers use the Lifesize Video Conferencing application which can be downloaded from your respective Apple or Google stores, or on your computer. You can also use the link below to join as a guest without creating an account.  

Our office staff will call you at the scheduled time of your appointment and provide you with the "extension" needed to reach  your provider. Please enter the patient's name in the name field.
